Answer for I have been using several types of diapers but they all seem to be reacting on my son’s skin who is barely 3 months. I have gone to an extent of putting a nappy liner before tying him the diaper. Please advise what could be wrong.

Some babies have very sensitive skin and the heat generated at the diaper area could be a culprit, firstly ensure that you change the diaper often, and immediately if the baby has pooped. Use a diaper barrier cream. You can also use a zinc & castor diaper cream.  You also need to rule out thrush so the next doctor’s visit, have him check any rash your baby may be having. If all else fails, you might have you use washable diapers that are made from more natural materials now found locally.
